  • Duffau’s stimulation series elicited semantic paraphasias along a ventral subcortical pathway that partly corresponded anatomically to IFOF. Those findings do not make EMC and IFOF interchangeable.

A patient says “cat” for a picture of a dog. Why should this not be reported only as “speech arrest”?

Explanation

Speech occurred, with a meaning-related substitution. Describe the semantic error and its reproducibility, then relate the localized observation to the relevant ventral anatomy. Speech arrest would conceal that task-specific information.

In a surgical discussion

During a dominant frontotemporal or insular case discussion, a semantic substitution is a different finding from a phonological error, articulatory failure or absent response. The error type helps define the anatomical question.

Compare the occipitotemporal ILF, the anterior temporal–frontal turn of UF, and the longer IFOF course. Identify each from its trajectory before discussing the naming task.

  • The named trajectories are anatomical references. Failure on a multicomponent task does not assign an exclusive function to the nearest tract.

Why can failure to name a picture be less localizing than a reproducible semantic substitution with preserved picture recognition?

Explanation

No naming response can arise at several stages, including perceiving or recognizing the picture, retrieving a word, initiating a response or producing speech. Preserved recognition and a characterized substitution narrow the question without identifying a single tract by themselves.

In a surgical discussion

A naming error should prompt characterization of the input and output components, not automatic attribution to semantics. Individual visual, recognition and language observations help interpret the lesion relationship.

Explanation

No. It changes the reference anatomy being displayed. Dominance and localization require individual evidence with an interpretable task and method; the left-sided lecture example establishes neither for a particular person.

For an individual case, retain the task, method, localization and performance beside any language claim. An absent observation and an observed negative result are different evidence states.
