Find a distributed network across the medial and lateral surfaces

Orient

The default mode network includes separated medial prefrontal, posterior medial and lateral parietal territories. Begin by following the red Yeo-7 wash around the surface, rather than looking for one central structure.

Rotate the atlas until you can name what lies in front, behind and beneath the displayed structures.

Compare

Start medially, then use the lateral view to find the parietal component. Return to the medial view and describe which territory lies anterior and which lies posterior.

  • The wash is categorical membership at one atlas scale. Red does not mean increased activation, metabolic demand or surgical risk.

Explain before revealing

Why would a lateral-only image give an incomplete anatomical account of this network?

Explanation

It underrepresents the medial prefrontal and posterior medial components. A network account needs both medial and lateral relationships, just as a lesion description needs its actual surface and depth.

In a surgical discussion

A medial frontal or posterior medial lesion can raise cognitive questions beyond the local motor and visual landmarks. Describe its anatomical level first, then specify the cognitive observation to be investigated.

Orient

Medial prefrontal and posterior cingulate/retrosplenial regions form an anterior–posterior relationship around the medial hemisphere. HCP 10r, 31pv and RSC are reference pointers within that broader anatomy.

  • Andrews-Hanna’s study identified anterior medial prefrontal and posterior cingulate regions as a core connecting other default-network components.
  • “Posterior medial cortex” is broader than any one posterior cingulate parcel; gyral, parcel and functional boundaries must remain distinguishable.

What does the label “posterior DMN hub” omit from a surgical anatomical description?

Explanation

Its actual cortical location and extent, side, depth and neighbouring structures. A hub designation describes a role in a network analysis; it does not supply an operative tissue boundary.

In a surgical discussion

For an interhemispheric or posterior medial case, distinguish the cortical target from the deeper callosal and cingulum relationships. Network terminology should add to that structural account.

  • Greicius combined functional and structural imaging to study medial prefrontal–posterior cingulate and medial temporal–retrosplenial relationships.

Explain before revealing

If two default-network regions have correlated resting-state signals, must one displayed cingulum streamline connect them directly?

Explanation

No. Functional coupling and a direct reconstructed connection are different observations. Common inputs and indirect relationships can contribute to correlation, and a displayed tract sample does not enumerate the entire network.

In a surgical discussion

A lesion near the cingulum can motivate a disconnection question even when separated cortical territories are preserved. That question requires actual lesion–pathway anatomy and cognitive observations.

  • In Andrews-Hanna’s experiments, the medial-temporal component was engaged when constructing memory-based scenes.
  • The midline core and the two subsystems were defined by that study’s connectivity and tasks, not by these HCP/subcortical labels.

Why is “DMN impairment” less informative than describing a specific difficulty with memory-based scene construction?

Explanation

The latter states a cognitive operation to assess and compare. The network label combines several components and tasks; it does not specify which ability changed or which anatomical relationship might explain it.

In a surgical discussion

Memory-oriented discussion of a medial temporal lesion should consider its connected cortical relationships. A single preserved hippocampal outline cannot summarize memory performance or connected tissue integrity.

Understand “default” without calling the brain inactive

Orient

The term arose from reproducible changes relative to baseline during tasks. It does not mean the network operates only when a person is doing nothing; internally directed cognition can itself be an active task.

Compare

Inspect the same DMN wash while comparing two imagined task instructions: attend to an external target, or construct a personally relevant future event. The colour stays unchanged because this is an atlas.

  • Raichle’s baseline measurements gave meaning to task-related decreases in selected regions.
  • Later experiments linked default-network components to self-relevant and memory-based cognition. A change in task contrast is not equivalent to a brain-wide on/off switch.

Explain before revealing

Does a region showing less signal during an externally directed task become anatomically irrelevant during surgery?

Explanation

No. Relative signal change answers a question about that task and baseline. It does not establish expendability, and it does not describe all of the region’s functions or connections.

In a surgical discussion

A task-related decrease should not be described as evidence that tissue is functionless. A cognitive account must state the comparison condition and the operation being studied.

Characterize the complaint against baseline and formal cognitive findings, then relate it to the actual lesion and connected anatomy. The atlas provides a hypothesis to examine, not a DMN-specific postoperative diagnosis.
